Breached records include names, social security numbers, health insurance information, Medicare/Medicaid numbers, medical record numbers, physicians’ names, diagnoses, test results, medical images, treatment information, and other patient care information.

If your identity theft monitoring service has notified you that your information was included in the breach, you are a VNS Health or Sharp Healthcare patient and believe your personal information has been stolen, or if you have received notification from Episource or your healthcare provider, then you may be entitled to compensation.
